What is Scene?
Through one-on-one medication adherence support, video coaching, and education, we empower your members with hypertension to improve their health and help health plans reach key metrics.
After at least 30 days in our program, health plan members:
- Achieve up to 98% medication adherence in as little as 90 days.
- Control blood pressure and capture blood pressure readings
- Identify and address an average of 2 medication challenges through a pharmacist-led CMR
- Identify and address an average of 7 medication, behavioral, and social determinants of health-related issues through daily video Directly Observed Therapy
- Give our program an average of 4.6/5 stars
